Piles are also known as hemorrhoids, which are swollen blood vessels in the bottom area i.e. anal canal. They are a spread out of the naturally occurring mesh-work of blood vessels lining the anal canal.

Hemorrhoids are classified into 4 grades / stages that range from mild to severe, with grade 1 being the mildest & with grade 4 exhibiting a swelling that remains outside the anal canal and cannot be pushed in.

Fistula

fistula is an abnormal connection between two hollow spaces (technically, two epithelialized surfaces), such as blood vessels, intestines, or other hollow organs. Fistulas are usually caused by injury or surgery, but they can also result from an infection or inflammation. Fistulas are generally a disease condition, but they may be surgically created for therapeutic reasons.

What Is an Anal Fistula?

An anal fistula is a tunnel that runs from inside the anus -- the hole your body uses to get rid of solid waste -- to somewhere in the skin around it. It usually follows an infection that didn’t heal the right way. Your doctor can repair the fistula, but you'll need surgery for that.

What Causes It?

Just inside your anus are several glands that make fluid. Sometimes, they get blocked or clogged. When that happens, a bacteria buildup can create a swollen pocket of infected tissue and liquid. Doctors call this an abscess.

If you don’t treat the abscess, it’ll grow. Eventually, it’ll make its way to the outside and punch a hole in the skin somewhere near your anus so the gunk inside it can drain. The fistula is the tunnel that connects the gland to that opening.

Fissure

Anal fissure or Fissure-in-Ano is a crack or injury on the soft and delicate lining of the anal canal. It is generally the result of passing a hard stool. These can appear at any age and are commonly seen in both the sex. Fissures as classified as

Acute – early, less than a month +/- 15 days

Chronic – long-standing, more than 2 months duration. Mostly have a skin covering like piles

 

Fissure Causes

Lateral Internal Sphincterotomy: It is associated with the same principle i.e, releasing the anal spasm but with a small muscle incised. It needs to be carried out by Fissures Treatment Experts only, as the degree of incised muscle directly correlates with bowel control.

Constipation

Constipation is a condition with an inability to empty one’s bowels. Generally, with those having less than 3 bowel movements per week. It occurs when the stool moves very slowly through the bowels making it very hard and dry.

The occasional occurrence of constipation is common but for a few, it becomes chronic and will interfere with daily activities. In chronic constipation, symptoms persist for several weeks.

What are the Symptoms of Perianal Abscess?

Throbbing and constant pain when sitting down is probably one of the most common symptoms of a perianal or perirectal abscess. This pain is accompanied by swelling in the anal area or buttocks and severe pain during bowel movements.

Other symptoms of an anal abscess include:

♦ Discharge of pus or bleeding near the anus.

♦ Fever and chills

♦ Fatigue

♦ Constipation

♦ A lump in the anal area

♦ Tenderness or itching of the skin around the anus.

How Perianal Abscess is Diagnosed?

Most anorectal or perirectal abscesses are usually diagnosed based on your medical history and a physical exam. If an abscess is located within the anal canal, your doctor may perform an endoscopy to look inside the anal canal and lower rectum.

Sometimes imaging tests such as computed tomography (CT) scan or magnetic resonance imaging (MRI) may be used especially if the abscess is deeper. During the physical exam, your doctor will want to make sure of whether the mass is an abscess or hemorrhoids.

Further tests may be recommended to ascertain that IBD or Crohn’s disease is not a contributing factor. In these cases, blood tests and colonoscopy, using a lighted and flexible scope to examine colon are required.

How Perianal or Perirectal Abscess is Treated?

Surgery is the standard treatment for anorectal abscess.

Anorectal abscess rarely goes on their own with antibiotic therapy. Because these abscesses are an indication of timely incision and drainage. In most cases, incision and drainage are performed typically in an office setting that involves a local anesthetic.

However, if the anal abscess is large and deeper or situated high in the rectum, the procedure may require hospitalization and general anesthesiologist. This procedure generally takes around 30 mins. After the procedure, antibiotics are prescribed to help treat the infection and pain relief. Patients may also be advised to use a sitz bath, to soak and clean the anal area.

During recovery, stool softeners and medications may be needed to allow the drained abscess to heal better. After every bowel movement, wiping the area gently with toilet paper and rinsing with warm water helps decrease the patient’s discomfort.

Once the perianal abscess surgery is done, the drainage of the abscess will provide almost immediate relief. Sometimes, pain and discomfort are usually mild and can be controlled by pain medications. Pilonidal Sinus

A Pilonidal Sinus (PNS) is a small cyst or abscess that occurs in the cleft at the top of the buttocks. A PNS usually contains hair, dirt, and debris. It can cause severe pain and can often become infected, in which case, it may ooze pus and blood and have a foul odor.

A PNS is a condition that mostly affects men and is also common in young adults. It’s also more common in people with a sitting job.

 

Pilonidal Sinus Treatment

There are several medical options for pilonidal sinus treatment. They are branched into two:

Colorectal Cancer

Colorectal Cancer (CRC), also known as bowel cancer or colon cancer, is the development of cancer from colon or rectum (which are part of the large intestine). Colorectal cancer (CRC) is the third most commonly observed cancer in men and the second most commonly observed cancer in women, worldwide. Although there exist wide geographical variations in incidence across the world, the geographical patterns remain similar in men and women, with almost 55% of the cases originating in developing countries only.

 

Mortality due to colon cancer is higher in the underdeveloped than in developed countries. This is because of poor health care systems and lack of awareness about the disease. The incidence of colorectal cancer treatment is relatively low in India due to significantly high fiber intake and a healthier lifestyle when compared with other countries.

 

Colon and rectal cancer are more frequently observed and diagnosed among people of age 65 to 74 years (median age is 68 years), though lately, it is visible in the lower age groups also. In the western world, colon cancer is observed more commonly than rectal cancer. In India, rectal cancer is more common with men being commonly affected than women.
